Use this form to nominate an OGIS Philippines colleague for the President's Award — the most prestigious recognition program in the organization.

Important Reminders Before You Begin

  • Self-nominations are strictly not permitted.
  • Contributions cited must fall within the evaluation period: February 2025 – September 2026.
  • Submit one form per nomination. Use a separate form for each award category.
  • This form must be approved by your Business Unit Head before submission to HR.
  • All nominations are strictly confidential and handled in accordance with company data privacy policies.

Guidelines for Strong Nominations

  • Specific — describe what the nominee did, not just who they are.
  • Evidence-based — include real examples, outcomes, and measurable impact.
  • Aligned — directly connect the nominee's contribution to the award criteria.

A short, specific, and compelling title — maximum 15 words. Example: "Rescued a Critical Client Engagement Through Decisive Cross-Functional Leadership."

Summarize your nomination in 100–150 words. What did this person do, and why does it deserve the President's Award?

Name the single most important thing you are recognizing. Be specific — one clear contribution, not a general description.

What is the specific timeframe of this contribution? Must fall within February 2025 – September 2026.

Name the team, department, client engagement, or project most directly affected by the nominee's contribution.

Section 5: Detailed Nomination Statement *

The heart of your nomination; tell the full story.

Address the guide questions below in your narrative — you do not need to answer them separately. Write in a way that makes the full picture clear to a reader who does not know the nominee. Recommended length: 300–700 words.

  • 2. What specific challenge, opportunity, or situation did they address?
  • 3. What actions did they take — and how did they approach it?
  • 4. How did they demonstrate OGIS Philippines' values of Teamwork, Excellence, Innovation, and Customer Focus?

  • 6. Who specifically benefited — and in what way?
  • 7. What makes this contribution stand out from typical performance?
  • 8. Why should this nominee be selected for the President's Award?

Section 7: Impact and Results

Provide numbers, metrics, examples, or evidence where available. Complete only what is relevant to this nomination.

How did this contribution affect business operations, efficiency, delivery, quality, or performance outcomes?

How did this contribution affect clients, partners, or external stakeholders?

How did this contribution affect colleagues, team dynamics, morale, or overall capability?

How did this contribution reinforce, advance, or strengthen OGIS Philippines' culture, values, or organizational identity?

Provide specific metrics, percentages, timelines, or quantifiable outcomes if available. Examples: cost saved, time reduced, customer satisfaction score, project delivery rate, revenue impact.

Name any managers, colleagues, clients, or stakeholders who can speak to the impact described above. Provide contact details also (email address, contact number).

List and attach documents that validate this nomination.

Attach any relevant documents that substantiate the nominee's contribution and its impact. Supporting documents should not include confidential client data, employee records, or sensitive financial information unless properly authorized for internal review. Examples of supporting documents:

    By submitting this nomination form, I confirm and agree to each of the following:

    1. I confirm that all information provided in this nomination is true, accurate, and complete to the best of my knowledge.
    2. I confirm that this nomination is based on actual contributions, behaviors, and achievements demonstrated by the nominee during the evaluation period (February 2025 – September 2026).
    3. I understand that incomplete, inaccurate, or falsified nominations may be disqualified from the program without appeal.
    4. I understand that submitted nominations, including this statement, supporting documents, and references, will be reviewed by HR, the Evaluation Committee, and/or the President of OGIS Philippines as part of the award evaluation process.
    5. I confirm that any supporting documents attached to this form are appropriate for internal review and do not improperly disclose confidential client, financial, employee, or sensitive business information.
    6. I consent to OGIS Philippines retaining and using the information in this nomination form for the purposes of evaluating, processing, announcing, and communicating the President's Award, in accordance with the company's data privacy policies.
